Thank you for contacting the Avoidant/Restrictive Food Intake Disorder - FEED Program at Boston Children's Hospital. We understand the difficulty of living with feeding difficulties and we sincerely hope we can help. This data collection system will provide us with background related to your child's medical history and chief complaint.

The data you put into the form will be stored securely on a server within the Children's Hospital protected network. While we may use the data we collect to further additional research, we will not share individuals' names or any other protected health information (PHI) that may identify the participants.
